The Diabetic Foot-Pain-Depression Cycle: Integrating Mental Health in Limb Preservation Depression remains significantly underdiagnosed among patients with diabetes, particularly those facing lower extremity complications. The shame, stigma, and social rejection associated with even "simple" toe amputations create profound psychological burdens that are often overlooked in clinical practice. Brandon Brooks, DPM, MPH, FACPM, FASPS, and his colleagues have identified an underlying Diabetic Foot-Pain-Depression Cycle that perpetuates both physical and mental health deterioration. This lecture explores the critical need for routine depression screening in diabetic foot patients, both preoperatively and postoperatively, emphasizing that clinicians should not hesitate to initiate mental health referrals when indicated. While non-traumatic amputations may appear routine from a surgical perspective, they represent genuinely traumatic experiences for patients. This presentation demonstrates why psychiatrists and other mental health professionals are essential members of comprehensive limb preservation teams, and provides practical strategies for breaking the destructive cycle that undermines both physical healing and psychological wellbeing.
